Vendor Information - Payment of Certification Fees

Dear Sir or Madam:
Pursuant to the Reduced Cigarette Ignition Propensity and Firefighter Protection Act, §53-7404(5) it is necessary to tender vendor information that may be required by companies, concerns or manufacturers for payment of Certification fees. The following information is given to assist in naming the “Utah State Fire Marshal” as a recurring vendor for payment:

Should there be a lag time between Certification application/list submittal and the payment of fees, it must be understood, as stated on the submission forms, “No brand styles will be Certified for sale in the State of Utah or be included in the Utah Fire Prevention and Safety Authorized List until the fee is paid in full.”
